To apply for an IVP Fingerprint Clearance Card:

Please visit the Arizona Department of Public Safety (DPS) website.

Arizona IVP Fingerprint Clearance Card: Step-by-Step Guide

Need an Arizona Identity Verified Prints (IVP) Fingerprint Clearance Card for employment, teaching, student teaching, licensing, volunteering, or another authorized purpose?

Follow these steps to apply through the Arizona Department of Public Safety and complete your fingerprinting appointment with Flagstaff Mobile Notary & Fingerprinting.

Before You Apply

Ask your employer, school, licensing agency, or sponsoring organization:

  • Whether you need an IVP or Non-IVP card
  • Whether you need a Regular or Level One card
  • Which employment or licensing reason/statute you must select
  • Whether you should apply as an employee or volunteer
  • Whether they will provide a payment or agency code

Arizona DPS and your fingerprint technician cannot choose these options for you. The organization requiring the card must provide this information.

Step 1: Create Your Arizona DPS Account

Visit the official Arizona DPS Public Services Portal.

Create an individual account or sign in to your existing account.

Make sure the name on your account matches your identification.

Step 2: Start Your Application

After signing in:

  1. Select Services.
  2. Select Fingerprint Clearance Card.
  3. Select Apply for a Card/Request a Replacement.
  4. Select Apply for a New Clearance Card.
  5. Choose the card type your employer, school, or agency told you to obtain.
  6. Read and accept the Privacy Act Statement.
  7. Select the correct reason or statute for your application.
  8. Select Employee or Volunteer, if prompted.
  9. Complete your personal information.
  10. Review and electronically sign the application.

Carefully review everything before submitting. Incorrect information may delay your application.

Step 3: Pay the Arizona DPS Application Fee

Choose the appropriate payment option:

  • Pay by credit or debit card
  • Enter an agency payment code
  • Request payment through your employer’s PSP account, if your employer has provided the required account information

The Arizona DPS application fee is separate from the $40 fingerprinting service fee. DPS fees are nonrefundable and may change.

We have the FD-258 cards, the affidavit and the envelopes.

Our equipment electronically scans your fingers and then prints the fingerprint images onto a physical FD-258 card. That is digital/Live Scan captured on FBI Appendix F-certified equipment and software, but it is not electronically uploaded for submission.

Step 4: Schedule Your Fingerprinting Appointment:

Flagstaff Mobile Notary & Fingerprinting
513 N. Beaver Street
Flagstaff, Arizona 86001
By appointment only
Call or text: 928-613-2206

Mobile fingerprinting may also be available by appointment.

Step 5: Bring These Items to Your Appointment

Please bring:

  • Your DPS application reference number
  • A current, unexpired government-issued photo ID
  • Any additional instructions provided by your employer, school, or licensing agency

Acceptable identification may include a state driver’s license, state identification card, passport, or other current government-issued photo identification.

The name on your identification should match the name used on your DPS application.

Step 6: We Complete and Submit Your IVP Fingerprints

During your appointment, the fingerprint technician will:

  • Verify your identity
  • Capture your fingerprints
  • Print the fingerprints on the required card
  • Add your DPS reference number where required
  • Complete the fingerprint technician section of the IVP affidavit
  • Place the fingerprint card and affidavit into the blue DPS envelope
  • Seal and mail the envelope directly to Arizona DPS, as required for Identity Verified Prints

For an IVP application, the completed fingerprint documents cannot be handed back to the applicant for mailing.

Step 7: Check Your Application Status

DPS states that electronic-application status information is generally available approximately 24–48 hours after fingerprints are received, although it may take longer.

You can check your status through the Arizona DPS Public Services Portal.

DPS will review the fingerprints and criminal-history results and determine whether the card can be issued. Your fingerprint technician cannot estimate approval, determine eligibility, or provide an exact processing date.

Renewing an Existing IVP Card

If you already have an IVP Fingerprint Clearance Card, you can apply for renewal through the DPS Public Services Portal.

DPS normally uses the fingerprints retained from your original IVP application, so you usually do not need another fingerprinting appointment.

You will need your original or “Master IVP” number.

In some cases, DPS or the FBI may require new fingerprints because of fingerprint-image quality. Schedule a new appointment only if DPS sends you instructions and a fingerprint packet.

Arizona Fingerprint Clearance Cards are generally valid for six years. DPS recommends applying for renewal approximately three months before expiration.

Important Information

Flagstaff Mobile Notary & Fingerprinting provides fingerprint capture and identity-verification services. We do not determine:

  • Which card type you need
  • Which statute or application reason to select
  • Whether you qualify for a clearance card
  • Whether DPS will approve or deny an application
  • How quickly DPS will process or issue the card

Please contact the employer, school, licensing agency, or organization requiring your card for help selecting the correct application options.

For application questions, contact the Arizona DPS Clearance Card Section at 602-223-2279.

For PSP technical assistance, call 602-223-2999 and select option 8.
