How to prepare for my fingerprinting appointment

Be prepared:

1. Required Documents:

  • Valid Photo ID:.You'll need a government-issued photo ID like a driver's license, passport, or state ID. 
  • Any Specific Forms/Instructions:.Check with the agency or employer requesting the background check to see if they require any specific forms or instructions to be brought to the appointment. 
  • Reference or Application Number (if applicable):.Some agencies may have a specific reference or application number that needs to be submitted with the fingerprints. 
  • Proof of Payment:.Depending on the situation, you may need to pay the fingerprinting fee. Acceptable forms of payment can vary, so confirm the accepted methods beforehand. 

2. Preparing Your Hands:

  • Moisturize: Apply oil-free moisturizing lotion to your hands several times a day, especially after washing, leading up to your appointment. This helps to create better print quality, especially if you have dry hands or shallow ridges. 
  • Avoid Harsh Chemicals: Avoid activities that can irritate or damage your skin, such as excessive handwashing or use of harsh chemicals, for a few days before the appointment. 
  • Avoid Alcohol-Based Hand Sanitizers: Alcohol can dry out the skin and make fingerprinting more difficult. 


Fingerprinting Appointment Intake Questions


  1. What agency or organization is requesting your fingerprints?
    (e.g., AZ DPS, Board of Nursing, Real Estate Licensing, employer, etc.)
     
  2. Do they accept ink card submissions (FD-258)?
    (Some agencies require Live Scan or electronic submissions only.)
     
  3. Do you already have the fingerprint cards, or will you need me to provide them?
    (You may provide FD-258 cards or advise where they can get them.)
     
  4. Do you have the specific instructions or forms from the requesting agency?
    (Some require specific codes, form fields, or return envelopes.)
     
  5. Is a reference number, ORI number, or any other identifying information required?
    (This is often needed for clearance cards or licensing.)
     
  6. What is the purpose of the fingerprinting?
    (Employment, licensing, clearance card, adoption, immigration, etc.)
     
  7. Will this be an in-office appointment or mobile service?
    (Get the exact address if mobile.)
     
  8. Do you have a valid photo ID to bring to the appointment?
    (Ensure they’re prepared.)
     
  9. Have you had issues with fingerprint rejections in the past?
    (If yes, you may need to take extra care with prints or note skin condition.)
     
  10. Preferred date and time for your appointment?
    (Helps with scheduling and confirmation.)
