IVP Applications for Education

To apply for an IVP Fingerprint Clearance Card:

Please visit the Arizona Department of Public Safety (DPS) website.


Select "Fingerprint Clearance Card Application, Requests, and Status Checks" for information on how to apply for your IVP fingerprint clearance card

For specific questions about obtaining, processing time, or renewing your IVP fingerprint clearance card please call the Arizona Department of Public Safety at 602-223-2279.  The DPS establishes the procedures for applying for an IVP Fingerprint Clearance Card.

 First, purchase a money order, cashier check or bring a business check made payable to ‘DPS’ (Employees = $67; Student/Volunteers=$65) -OR- apply online at psp.azdps.gov, pay the DPS pay and select ‘paper fingerprints” (do not prepay for fingerprinting). 

You do not need to order cards and envelopes. We have the needed supplies available.

If you apply online, please have your reference number when you arrive.  


 Make an appointment for fingerprinting and let the us know you need an IVP Fingerprint Clearance card paper application or you have already paid online and have a reference IVP application number.


 IVP Paper Application: All personal information on the application must match what is on the fingerprint card and then signed and dated. The associate will complete the bottom portion of the IVP application, provide you with a receipt and send your completed packet to DPS in a blue, prepaid envelope. 

-OR-  

 Online Application: write the reference number on the fingerprint card in the MNU field with a black pen. The fingerprint technician will send your completed fingerprint card, and an IVP Chain of Custody form to DPS in a blue, prepaid envelope.   


The person performing the fingerprinting  shall compare the photo identification to the applicant.   The fingerprint technician shall also compare the physical descriptors and personal information on the applicant's photo ID against the information on the application form and the fingerprint card fingerprint card to ensure the information matches the applicant.   The person taking the fingerprints must complete the “Attention Fingerprint Technician” section of the application form.  This is the chain-of-custody section: if this section is not completed, the application packet will be returned unprocessed.  The person taking the fingerprints CANNOT return the fingerprint card to the applicant once the applicant has been fingerprinted. The person performing the fingerprinting shall then place the application form, the fingerprint card, and the payment (cashier’s check or money order if not already paid online and reference number provided) in the blue DPS postage-paid return envelope and place the envelope in the mail.   The envelope with the fingerprints CANNOT be given to the applicant. 


Once your prints have been taken on the provided cards, the technician who took the prints must close the prints and the completed affidavit in the blue envelope and they must mail it back to DPS.


 Once AZDPS receives the print cards  the fingerprints will be processed for review. It is imperative that the Reference Number be included with the fingerprint card. You can write the Reference Number on the paper card in the box labeled MISCELLANEOUS NO.

Educator Certification: Fingerprint Clearance Card (IVP)

Arizona Department of Public Safety Fingerprint Clearance Card

All teachers and individuals that work in the classroom are required to have an Identity Verified Prints (IVP) fingerprint clearance card per Arizona Revised Statue 15-106.

Important!

  • Be sure to request the Identity Verified Prints (IVP) application for Teacher Certification when contacting DPS.
    • Be sure to mark Teacher Certification as the sponsor on the application.
    • Include your SSN with your IVP Fingerprint application.
      • These 2 steps will authorize DPS to share your information with the Certification Unit and allow you to create your account on the AzEDCert portal.
