1. Bring the Following to Your Appointment:
- A valid government-issued photo ID (driver’s license, state ID, passport)
- Two (2) FD-258 fingerprint cards (if not provided by technician)
- ATF form you're applying with:
- Form 1 (Making NFA item)
- Form 4 (Transferring NFA item)
- Form 7 (FFL application)
- Form 5400.13/16 (Explosives permit)
- Completed ATF Cover Letter (if using eForms and sending fingerprints separately)
- Names of trust/corporation members (if applicable)
2. During Appointment:
- We will:
- Properly roll your fingerprints on FD-258 cards using black ink or Live Scan printout
- Ensure all fields on the fingerprint cards are filled, including:
- Full name
- Date of birth
- Place of birth
- Sex, race, height, weight, eye & hair color
- Address
- Reason for fingerprinting (e.g., “ATF NFA Form 4”)
- ORI number (typically WVATF0800 for NFA)
- We will check the fingerprint quality to minimize risk of rejection
3. After Appointment - What You’ll Do Next:
For eForms (Form 1):
- Submit the application online at eforms.atf.gov
- Print the cover letter you receive after submission
- Mail the cover letter + 2 fingerprint cards to:
ATF-NFA Division P.O. Box 5015 Portland, OR 97208-5015
For Paper Forms (Form 4, FFL, etc.):
- Include fingerprint cards in your application packet
- Mail the full package to the appropriate ATF address listed on your form
🕒 Processing Times (Estimates)
- eForm 1: 30–90 days
- Form 4 (paper): 8–12 months
- FFL/Explosives Licensing: 4–8 weeks after fingerprint processing
🔒 Important Notes:
- Keep your receipt for proof of fingerprinting
- Double-check your application for errors before submitting to ATF
- If submitting for a trust, each responsible person must submit 2 fingerprint cards + a passport photo
Fingerprints must be submitted on Fingerprint Identification Cards, FD-258 that have been issued by ATF. The fingerprint cards must contain the following ORI information: WVATF0900; ATF-NATL EXPL LIC, MARTINSBURG WV.
