Step 1: Complete Your Application
- Visit the Arizona DPS website: https://www.azdps.gov/services/public/fingerprint
- Select the correct licensing or clearance application (e.g., IVP, Non-IVP, Security Guard, etc.)
- Complete the application form associated with your reason for fingerprinting
- Print and sign the application if submitting by mail
- Pay online. Bring reference number from receipt as it has to be included on the cards and affidavit submitted.
Step 2: Get Fingerprinted
- Schedule an appointment with Flagstaff Mobile Notary & Fingerprinting
- You will be fingerprinted on an FD-258 fingerprint card (2 sets should be submitted)
- Pay for fingerprinting services
Step 3: If Sending Payment
Pay the DPS fingerprint background check fee using one of the following:
- You can complete and pay application online. Bring reference number to appointment or send
- Money order
- Cashier’s check
Payable to: Arizona Department of Public Safety
Personal checks or cash are NOT accepted
Step 4: Mailing Your Packet
The Fingerprinter has to fill out an affidavit and seal the following items in one envelope:
- Completed application form if not done online
- AZDPS Affidavit
- Original fingerprint cards (do not fold)
- Payment (money order or cashier’s check)

Step 5: Wait for Processing
- Processing time can take 4–8 weeks, depending on DPS workload

Tips:
✅ Use black ink on all forms
✅ Make sure all information is clear and legible
✅ Do not bend or fold the fingerprint card
✅ Keep a copy of your money order/cashier’s check receipt for your records
✅ If the card is rejected due to poor quality prints, you will need to re-submit a new card
