AZ Liquor License DLLC

Fingerprinting for Liquor License

  • Apply  for a liquor license through the Arizona Department of Liquor Licenses and Control (DLLC).
  • DLLC sends fingerprinting instructions if the applicant needs a background check. This usually includes:
    • The ORI number
    • Fingerprint card format (usually FD-258)
    • Where to send the completed fingerprint card(s)
  • Client brings the fingerprint request and ID to

 Flagstaff Mobile Notary & Fingerprinting.

  • They will verify your identity and collect prints (ink or Live Scan-to-card depending on requirements).
  • You fill out the FD-258 card:
    • Include ORI, reason for fingerprinting, name, DOB, etc.
    • Roll all ten prints and two thumbs
  • You receive the completed fingerprint card  AND completed Fingerprint Verification form provided by the AZ Liquor dept in a sealed envelope to mail off. 
    • mail it to the agency or the agency collects it directly depending on their instructions.

  • Provide pre-printed FD-258 cards with common ORIs (if legal in your area)
  • Mailing service (mail card to the agency for the client)
  • Notary service if documents need to be notarized as part of the application

📬 Where the Fingerprint Card Is Sent

For most liquor license applicants in Arizona, fingerprints must be submitted to:

🔸 However, sometimes DLLC may give specific fingerprint instructions depending on the license type. Did you receive a Request for Fingerprint Submission or ORI information sheet from DLLC.
