Fingerprinting for Arizona ADHS Medical Marijuana Program
We make the fingerprinting part of your ADHS Medical Marijuana application simple, fast, and compliant.
Who needs fingerprints?
Per ADHS, fingerprints are required for:
- Designated Caregivers (including custodial parents/legal guardians for patients under 18)
- Dispensary Agents (board members, employees, contractors, volunteers)
- Laboratory Agents (owners, employees, contractors, volunteers)
- Facility Agents for adult-use establishments (officers/board, employees, contractors, volunteers)
Qualifying patients do not submit fingerprints—only the roles above do.
What we do for you
- Roll two sets of ink fingerprints on FD-258 cards (ADHS requests two originals to avoid FBI/DPS reprint delays). Verify your identity with a valid, unexpired government-issued photo ID.
- Complete and sign the ADHS Fingerprint Verification Form and seal your packet across the flap as required.
- Hand the sealed envelope back to you to mail—or, if you prefer, we can mail or courier it to ADHS for you (see addresses below).
What to bring
- One valid, unexpired government photo ID (driver license, passport, etc.).
- Your application type (Caregiver, Dispensary Agent, Laboratory Agent, or Facility Agent) so we label everything correctly.
- (Optional) Any ADHS paperwork you’ve already started—we stock FD-258 cards and the Fingerprint Verification Form if you need them.
How the ADHS mailing works
ADHS requires you to submit two original fingerprint cards + the Fingerprint Verification Form in a sealed envelope. Packets are mailed or delivered to ADHS at one of these locations (don’t send to DPS or the FBI):
- By U.S. Postal Service: ADHS, Attn: Marijuana Dept, P.O. Box 19000, Phoenix, AZ 85005
- By FedEx/UPS or other courier: ADHS, Attn: Marijuana Licensing, 150 N 18th Ave, Phoenix, AZ 85007
- In-person drop-off (M–F, 8–5, except state holidays): 150 N 18th Ave, Phoenix, AZ 85007 (visitor parking on lower level of garage)
Cards must not be folded or creased
